| Abstract |
Oculopharyngeal muscular dystrophy is a highly prevalent disease in the province of Québec, affecting 1 in 1,000 people. To date, there are few comprehensive and synthesized resources reporting the characteristics and functional impacts of swallowing and speech disorders associated with this disease. This study aimed to document these characteristics and functional impacts. A systematic review of the literature was conducted on oculopharyngeal muscular dystrophy and its characteristics between July and November 2020. The QualSyst Tool was used to assess the articles’ methodological qualities. Twenty articles were selected and data on swallowing or speech characteristics associated with oculopharyngeal muscular dystrophy and their functional impacts were extracted from them. Pharyngeal swallowing impairments were reported in most cases. Speech was characterized by reduced vocal loudness, dysphonia, hypernasality, and some articulatory difficulties. These speech characteristics reveal that speech disorders associated with oculopharyngeal muscular dystrophy match a clinical presentation of speech disorders of structural origin characterized by hypotonia and muscle weakness. This study is a useful tool for guiding the practice of the various professionals likely to work with clients with oculopharyngeal muscular dystrophy. La dystrophie musculaire oculopharyngée est une maladie à forte prévalence au Québec, atteignant 1 personne sur 1000. Il existe, à ce jour, peu de ressources récentes, complètes et synthétisées rapportant les caractéristiques des troubles de la déglutition et de la parole associés à la maladie, et à l’impact fonctionnel de ces caractéristiques chez les personnes en étant atteintes. L’objectif de cette étude est de documenter ces caractéristiques et leurs impacts fonctionnels. Une revue systématique de la littérature sur la dystrophie musculaire oculopharyngée et ses caractéristiques a été effectuée entre juillet et novembre 2020. La qualité méthodologique des articles a été évaluée à l’aide de l’outil QualSyst Tool. Au total, 20 articles ont été retenus desquels les données portant sur les caractéristiques de la déglutition ou de la parole, ainsi que leurs impacts fonctionnels ont été extraites. La présence d’une dysphagie pharyngée est majoritairement rapportée. De son côté, la parole est caractérisée par une intensité vocale réduite, la présence d’une dysphonie, une hypernasalité et certaines difficultés articulatoires. Les caractéristiques ressorties mettent en lumière un tableau clinique de trouble de la parole d’origine structurelle caractérisé par l’hypotonie et la faiblesse musculaire. La présente étude constitue un outil de grande utilité pour orienter la pratique des différents professionnels susceptibles d’oeuvrer auprès de la clientèle atteinte de la dystrophie musculaire oculopharyngée. |
